This app helps collect customer satisfaction data through a star rating visual interface. You can use it as a starting point to quickly create forms for rating ecommerce orders, customer support interactions or any other feedback scenario.

The app works with any of the native HTML form elements (text inputs, checkboxes, etc) but also supports two custom inputs: a star rating selector and a slider for picking satisfaction levels.

Customization

You can delete and change the form fields as you see fit. When customers fill out the inputs and submit their choices, the Smart Forms integration will handle everything and send an email with the collected data.

Get started

Download the .bsdesign file from the button above, and open it in Bootstrap Studio to adapt it for your specific use case. You will then need to select the form and setup the Smart Forms recipient.
